I am giving VegetarianTeen.com my consent, as Legal Guardian of mentioned child to contribute to your website, including emailing, researching, writing, interviewing, etc. I understand that my child's privacy will be protected. VegetarianTeen.com will never print my child's contact information or photo without a separate parental consent form addressing the specific issue. Email links available on the site will be for the adult staff of VegetarianTeen.com only. Any email addressing my child will be filtered through staff and then forwarded to my child, or if I request, to me. My child will have no direct contact from site visitors. If I, or my child, choose, all his/her work may appear under a pseudonym.
I understand that my child retains copyright of all writings and may reprint at any time with or without compensation and without obtaining permission from VegetarianTeen.com. However, we agree not to publish, permit to be published or otherwise take any action with respect to the publication of the writings, whether in print, electronic or other format without including original publication information. (For example: First published on VegetarianTeen.com, Jan/Feb 1999).
By checking here _____ I give permission for my child to review books, videos and/or products (such as food or clothing) from vegetarian companies at my discretion. I understand and agree to the following:
BOOKS
- My child may keep any book for which he/she completes a review that is published on vegetarianteen.com
- Book reviews are expected within two months of receiving the book, but extensions are permitted upon request
- If my child decides not to complete the review, I agree to return to the book to an agent of vegetarianteen.com at my expense.
VIDEOS
- All videos should be returned to an agent of vegetarianteen.com within 30 days at my expense, but extensions are permitted upon request.
PRODUCTS
- Product reviews are expected within two weeks of receiving the product, but extensions are permitted upon request
- If my child does not like the product, or I feel it is inappropriate for review, I will notify an agent of vegetarianteen.com as soon as possible with detailed reasons why the review will not be submitted.
- Because companies provide products in the hope of receiving a positive review, I understand that vegetarianteen.com's reputation is at stake and that either completing the review for publication or submitting an honest negative review to vegetarian.com staff is of utmost importance.
- I understand that if my child does not complete the review, and neither I nor my child provides vegetarianteen.com with reasons why he/she cannot complete the review, agents of vegetarianteen.com will have to pay for the products.

Your child has indicated an interest in contributing to the production and maintenance of VegetarianTeen.com. The purpose of this website is to give vegetarian and vegan teens the opportunity to share, learn, and meet other teen vegetarians while contributing to the content of the site. Your child's contributions to the site may include personal stories, articles, research, and interviews with other vegetarians. We feel, however, that a child's first priorities should be school and family. We at no time expect your child to take away from these activities/people to contribute to VegetarianTeen.com.
It is of the utmost importance to the team of VegetarianTeen.com that your child remains safe while having the opportunity to be published and participate in this exciting venture on the Internet. We will not publish any contact information about your child. All correspondence from outside the site will first go through staff. Email links available on the site will be for the adult staff of VegetarianTeen.com only. Any email addressing your child will be filtered through staff and then forwarded to your child, or if you request, to you. Your child will have no direct contact from site visitors via email.
We take your child's work very seriously and we will be available in the future for references, both verbal and written for site contributors.
